This skittering creature resembles a small, leafless, walking plant. Twig blights hide by rooting themselves among ordinary plants.

False Appearance. The blight resembles a dead shrub. While it remains motionless among vegetation, it can hide without being out of sight.

False Appearance. While the blight remains motionless it is indistinguishable from a tangle of fines.
Constrict. Melee Weapon Attack: +4 to hit, reach 10 ft., one target.
Hit: 9 (2d6 + 2) bludgeoning damage, and a Large or smaller target is grappled (escape DC 12). Until this grapple ends, the target is restrained, and the blight can't constrict another target.
Entangling Plants (Recharge 5-6). Grasping roots and vines sprout in a 15-foot radius centered on the blight, withering away after 1 minute. For the duration, that area is difficult terrain for nonplant creatures. In addition, each creature of the blight's choice in that area when the plants appear must succeed a DC 12 Strength saving throw or become restrained. A creature can use its action to make a DC 12 Strength check, freeing itself or another entangle creature within reach on success.

Grasping Roots. The area within 15 ft. of the Blighted Heart is difficult terrain for enemies as roots shift and trip them.
Rooted Presence. The Blighted Heart can't move. Its limbs and vines reach out from the place it sprouted.
Multiattack. The blighted heart makes two branch slam attacks.
Vine Pull (Recharge 5-6). Each creature of the blighted heart's choice within 25 ft. must make a DC 12 Strength saving throw or be up to pulled 10 ft. closer to the heart, and have speed reduced to 10 ft. until the end of their next turn. If a creature ends its turn within 5 ft., it takes 3 (1d6) bludgeoning damage from vines holding them.
Branch Slam. Melee attack: +5 to hit, reach 10 ft., one target.
Hit: 6 (1d6 + 3) bludgeoning damage.
The blighted heart can take 1 legendary action. The blighted heart regains spent legendary actions at the start of its turn.
Summon Blight Spores.. If there are fewer than 4 blight spores present, summon 1d4 - 2 (min 0) blight spores to unoccupied spaces within 20ft. of the blighted heart. For each spore summoned in this way, the blighted heart loses 2 HP.
